Memorial and Tribute Submission Form

This form does not immediately post, but provides information to our volunteers to create a memorial or tribute.  We will match submitted stories with their corresponding donation.  Here are some guidelines:

1. Title should include the name of the cat or human receiving the memorial or tribute.  Include whether it is a memorial or tribute, e.g. ‘In Memory of Mr. Purrrfect.’
2. The body of the post should contain the text of the memorial or tribute.  It can be as long or short as you wish.
3. Please provide 1 or 2 photos of the one being remembered.
